GM Case - 3

   March 4,2023. Case scenario.. Hi, I am Pranavi Sripada, student of BDS - III. This is an online elog book to discuss our patient’s health data after taking his/her consent. This also reflects my patient centered online learning portfolio. CASE SHEET A 65 year old female, resident of Konugodu has presented with Chief complaint: Shortness of breath , chest pain , swelling of face & legs since 10 days. History of Present illness: The patient was apparently asymptomatic  2 years before She went to a hospital with NYHA grade-1 shortness of breath with no associated fever,sweating & pain. There she was diagnosed hypertension. 10days back She complaints of breathlessness which is gradual in onset , progressive from grade - II to grade - III associated with chest pain radiating to the left side of the neck & left shoulder & left arm and She was having swelling of her face & feet. History of past illness: Hypertension since 2 yrs  (On medication tab.atenolol 50mg + tab.am

GM Case - 2

February 27,2023.. Case scenario.. Hi, I am Pranavi Sripada, student of BDS - III. This is an online elog book to discuss our patient’s health data after taking his/her consent. This also reflects my patient centered online learning portfolio. CASE SHEET A 72 year old female, resident of Chityala has presented with Chief complaint: Shortness of breath & chest pain since 2 days. History of Present illness: The patient was apparently asymptomatic 2days before. When she woke up in the morning she was having sudden onset of grade- IV shortness of breath associated with right side chest pain which is radiating to right side of the neck above & to right side of the abdomen below. It is also associated with profuse sweating. The patient had a heart stroke & a stent was placed in her heart 4 years ago. History of past illness : The patient is non-diabetic and her Blood Pressure is normal. Family History - Nil. Patient History : Diet - Mixed Appetite - Decreased  Bowel & Bladder

February 13,2023. Case scenario… Hi, I’m Pranavi Sripada, student of BDS-III. This is an online elog book to discuss our patients health data after taking her consent. This also reflects my patient’s centered online learning portfolio. CASE SHEET: A 75 year old woman with stomach pain, fever with chills, drowsiness, dizziness, weakness. Chief complaints: Fever with chills , stomach pain since one week (7days). Drowsiness, dizziness,weakness since 4 days Patient was apparently alright before 1week. History of present illness: Since one week she is having high grade fever with chills, intermittent lower left abdominal pain,weakness, drowsiness & dizziness because of decreased food intake. Past history :Nil  Family History : Not significant Personal history:     Diet: mixed     Appetite: loss of appetite     Bowel & Bladder : regular    Sleep : inadequate  General examination:  Pallor - Yes Icterus - No Cyanosis - No Clubbing - No Lymphadenopathy - No Oedema - No Provisional diagn
